How many LOR to assign?

I have been reading so many mixed reviews about assigning 3 vs. 4 LOR (including IM chairmans letter) to different programs. What is the most appropriate? Does everyone just blanket send 4 LOR to programs? Right now I've just assigned either 3 or 4 depending on the wording of the program's website - but I can't find a consensus on this topic anywhere.

Any program that cares about whether you give them exactly three letters or a fourth extra is an incredibly petty one. I'm sure in real life nobody cares

Maybe that 4th LOR was written by some PDs favorite former Chief Resident. Maybe it was written by some PDs mentor. Maybe it says something really spectacular about you.

Assign 4 unless they say "no more than 3".
