How to cancel an acceptance?

I feel horrible doing this so I would like opinions on what is the best way to do this.
Phone, email, in person? How to word such a letter?

Just do it by phone. You're not going to be going there, so it's not like you need to save face.

If you guys can't figure this out, I would NOT trust you as pharmacists.

You will probably need to write an email. Most schools I talked to needed my declination of acceptance in writing for their records.
