Get a kids' mental math app. I did that before I even started practicing problems...just downloaded something called 'King of Math' on iOS and plugged away. It's fun AND vaguely useful!
...but yeah, that doesn't take a forum poll to figure out. Or it shouldn't, anyway.
Math apps (per @mehc012), Khan academy math, rudimentary math textbooks, online math practice/review (again, elementary arithmetic and algebra), or make up your own questions and check yourself with a calculator.
she has a bunch of "math without a calculator" tutorial videos that are really helping me. I was having the same issues--I knew the concepts and equations but then would get tripped up by the math (physics and any form of math was ~6 years ago for me), which just made me feel dumb as rocks.
