How to make concise notes

This question might be dumb but I'm trying to build a little repository of diseases on my phone that I encounter during rounds/shadowing experiences and I'm a little lost with how to make everything concise. Anyone have tips? For example for a rare disease like Acute Disseminated Encephalomyelitis, how would you make a concise write up for a notes app?

I remember first seeing this 1-2 years ago and thinking it was so incredibly beautiful.

OP -- I would just have a few lines about pathophys, clinical sx, work up, and plan. With emphasis on work up and plan. I would also emphasize the things you will likely see again, like how to manage a COPD or CHF exacerbation, not acute disseminated encephalitis
