How to reply to a program?

And the sooner the better. Programs, for whatever reason, seem to send out more invites than they have slots, so it really is first come first serve.

It may not be true of every program, but some of the invite emails say it explicitly. And I've had at least one classmate this year wait too long and get put on the waitlist for an interview because the dates were all full.

My question is, what is the etiquette for canceling an interview? And do they really care if you cancel as long as you give them a good amount of lead time?

There is no reason to wait for any amount time. You should respond immediately, preferably within a few hours of the invitation. Don't be surprised if your date preference fills during that time period.

If you are canceling an interview, just try to give them as much notice as possible. They don't care if you cancel, as long as you give them enough notice to fill the spot. It is bad etiquette to the program and to your fellow applicants to wait too long. Both parties may lose out on a chance to match.
 
I wouldn't email them...I'd call. ASAP.

I think the longest downtime I had between interview invite and having called and confirmed was about 5 hours (and that was because I was scrubbed in when the email came...I called back from the OR hallway while we were waiting to transport the patient to recovery). And even with that degree of OCD I wasn't able to get my #1 choice of dates at every program.

If you want to re-schedule, maybe include the reason why you're canceling (if it's a good reason), but if you're just canceling and withdrawing your application, say "Hi, thanks for your consideration, I'd like to withdraw my app and cancel my interview. Sincerely, Seward"

Be polite, but no reason to make it any longer than one or two sentences.

After you email them the date? Do they respond back usually?

Usually they do respond back. Some soon (minutes). Some the next day. Some might even take a day or two. But if you're concerned, you may want to re-contact the program just to make sure they received your message.
