How to study medicine from first principles?

How do I do this? I want to do this and be at the point where I can ranodmly choose a part of the body, cause something to go wrong and reason what will happen?

Does reading Robbins gives me a first principle understanding behind pathology and thats good enough for the basic science side? I feel like basic science is important to actually understand whats going wrong. But the way med school teaches basic science is way too much memorizing over understanding
 
in medicine you have to read a lot, but at the same time understand what you read. the more you learn and know, the easier it will be to reach a high level
