IA Official Documentation?

I just read on Temple's website that they require an official document detailing IA's sent to them from your school if you've had an institutional action. This is the first I've heard of this. Is this the case for most schools? Does anybody have a list of the schools that require it?

All schools check to see if you have IAs, particularly after acceptance. Due diligence and all that.
