I am more worried about the chaos that would cause. Suddenly all these physicians have no pay checks (especially for prior month’s wages), no medical malpractice coverage (current and past), and no benefits (some). It would take 1-2 months of negotiations with hospitals, medical coding/billing companies and malpractice insurance, and finding new benefits. It would be hell...

I am sorry but I am just a punch in and punch out kind of doc. Would/could I do all of the above? Yeah, but it would be hell. And I am tired.

Any groups only needs a small trustworthy handful. The issue is many people become overtaken with power and ckntrol and lose focus on helping their partners. On the other hand many partners refuse to pay a reasonable wage for other docs to do admin work. This in my experience is a pitfall of sdgs and must be continually addressed and worked on.

Agree, I've seen both. Our COO and CFOs both make six figures less than what our docs make. Our CEOs have typically made less than many of our specialists. I'd gladly take on the CEO job for a raise, but not a paycut, which is what it would be for me in my group.

Same. In my experience the EM group leaders work for about 2/3 the wage of the worker. I think this makes sense as most admin work is M-F and much of it like meetings and such are done during normal business hours. This is not an absolute but at a minimum you have to work fewer nights and weekends which I view as a positive.

The struggle is keeping rates up for admin as the group grows or incomes grow.
 
It might be a buying opportunity for someone sitting on the sidelines wanted to get into the healthcare biz. Maybe buy low run lean. Distressed office practices might find the PE money appealing.

The unfortunate reality is that (a) they'll either just CH 11 and keep going or (b) some combination of TH, Schumacher, and USACS will split up the contracts. Less competition will equal higher costs for hospitals, higher costs for patients, and smaller paychecks for us.

Get some friends together and approach your hospital CEO/CMO/CFO etc.

AAEM has a 20ish page primer on forming a group, doing the RFP, etc. Found it by Google search recently. Not sure what it was called. I think I searched "how to form an emergency medicine group."
