To obtain a preliminary surgery spot it's not very hard at all. Many go unfilled.

It's becoming increasingly competitive with more and more US trained students entering the match without an increase in the number of residency positions.

However, the crux of the problem is mentioned in your quote above and that is finding a full training position and not just an intern year can be difficult.

I think it is better to a get a preliminary position. It will give you a chance to show yourself and perhaps stand out from crowd.

Better than a categorical spot? Nope.
