No, there isn't a grand list. You simply input whatever the BIN, group, and ID numbers are on the card. There may be a PCN number as well, and for certain insurances it is necessary.
You will quickly learn that 004336 is Caremark, 003858 is Express Scripts, and 610014 is Medco, which is now Express Scripts. Everything else is what it is, and insurances change year after year, with brand new BIN numbers.
