Is it possible to have the patient pay up front and then submit a claim to their insurance companies on their own? Has this ever been done at a clinic you have worked at?

How much does Medicaid and Medicare cover for eye exams? How much goes into your pocket?

For insurances we didn't take at the office I worked, the patients paid up front and then filed for reimbursement on their own by filling out a form usually on the provider's website. It was only a percentage, but they got something back.

Medicare if I remember correctly (it's been a while!) has around a 15% copay depending on if the patient has a supplementary insurance, etc. I don't know how much reimbursement is though.

Depending on the insurance plan, patients can go "out of network." This would mean you don't accept the plan but the patient still wishes to see you. They pay you and then will get a percentage reimbursed. Their out of pocket cost to see you will be much higher than to see an "in network" doctor.
