Insurance Coverage for Botox?

Has anyone been able to get insurance to pay for Botox trigger point injections? I have a lady who has Parkinson’s who has developed significant tightness and myofascial pain in her buttocks and thighs. Her neurologist recommended Botox to those places... They are willing to pay out of pocket, but I am trying to get their insurance to cover it. What ICD codes would work for this, if any??? Also, how many units would you inject into these kinds of muscle groups?
