I guess in the grand scheme of things AOA doesn't really matter. More like board scores, letters, and being a good person.

AOA is very important for some programs in certain specialties (I've heard some ortho programs only invite AOA applicants to interview, for example. Obviously they make exceptions for schools without AOA though.)

There's "no internal ranking" where they tell you there's no rank yet they actually rank you and keep your "shadow grade" that's used solely for AOA.
Then there's schools who actually do not keep a rank and do not apply pre-clinical years to AOA.

Yeah, they'd have to make exceptions - otherwise they'd be getting no HMS or Stanford med students.
