Was it bad form to have let myself out of the doctors office in which I was interviewing instead of being offered the door?

Does it look like I couldn't get out of there faster?

Unless you didn't shake their hand and say something like "thanks for your time", or "pleasure to meet you"...or something else along those lines...you should be fine. I wouldn't worry about it too much.
 
I really doubt that something like that would make or break the impression he got of you during the interview. I'm sure you're fine.
