Is EM CCM competitive?

I'm an intern at a Chicago EM program and am very interested in CCM. It seems more and more programs are opening their doors to EM applicants (I think http://www.emccmfellowship.org/ is missing several places).

The SF Match site doesn't provide much data, but it seems ~90% match. I either want to stay in Chicago or go to a top program elsewhere. Does this require a lot of planning ahead of time? For example, do I need to start getting heavily involved in CCM research, go to conferences, make connections early? Or, can I wait until my PGY2 year to start figuring this out?

All top programs for anything( even renal and id) are competitive.
It will not hurt him( and will enrich him plenty) to prepare early and get involved and show an interest early on....
I could go on and on.... Should I?
 
If you know more, yes. Specifically, is the current CC landscape such that EM folks have to fight tooth and nail for a spot at a good program? Is it relatively easy given sustained interest through residency? Somewhere in between? In general what is the competitiveness of CC fellowships?

It is definitely possible to secure an EM-CCM position somewhere, particulary with more programs opening up and IM-RRC apparently dropping their 1 out 4 rule. The top programs, particularly those with a strong EM-CCM record, however certainly can pick and choose from a pool of very talented applicants. It does help, however, that there is no match, so even the good programs will often extend an early offer if they are impressed by an applicant.

Is there an unsung rule about EM folk who have been out of residency for a few years but want to pursue CCM? Is this feasible or is it basically apply right out of residency? Or is it too early within the EM-CCM history to give a good answer here?

Too early. It can be done by either route (straight out of residency, or after some time in practice).
But like any fellowship, it's tough to get out of residency, be out for a while, then return to training. CCM is worse than others because the fellowship hours are so different than private practice hours. Cut your income by 3/4, double (or triple) your hours worked, and give up the autonomy of being at the top of the hierarchy...
