Let's say that you want to apply to EM programs, but you only want to apply to ones nearby--and you want to keep (nearby) family practice as a backup.

Can you submit a rank-order list to NRMP with the 3 or 4 nearby EM programs (assuming you've interviewed there), and have the FP ones ranked after that? Or do you have to submit it with all one type of specialty on it?

People apply for two residencies all the time. You can rank an EM, FP, IM, and then another EM. The difficulty comes in securing two sets of LORs. You would also have to write different personal statements.
