Is 'out-of-match' a thing of the past?

Okay, things haven't been bright, and, as an IMG, I'm planning to focus the few programs that may still offer prematches...most likely for the next season. I can't seem to find them. The few on the Freida list don't seem the types that would want IMGs.
 
Because of the "all in" rule, the number of hospitals that offers pre matches has dropped pretty significantly. Your best bet is going to be less desirable programs in less desirable locations at community hospitals without multiple specialty residencies. Going through the match and taking whatever you can through soap might actually be higher yield.

It seems like it has dropped to zero for IMGs. No, SOAP is going to be just another ordeal. There is no way on earth anyone is going to bother calling an unmatched IMG who is not even in the U.S. and offer to sponsor their visa when there will be plenty of better looking applicants.

I feel like my only shot at any success can be by personally making myself known at the lowest tier program through an observership. The problem is I can't find such a program.

out of match programs will typically be all I/FMGs so not sure what programs you did find that don't look like they take foreign grads...US seniors cannot take a prematch.
as the years go on, the number of programs doing everything outside the match will dwindle.

If you are considering applying for residency, you will need to apply to a LOT of programs, be willing to go to ANY interview and rank as many programs as you can,

the SOAP is not the only ordeal you will need to be willing to endure…the whole process is a bit of a one, but it is one you will need to go through.

as you said, no one is going to just hand you a residency spot….you are going to have to work for it…applying to a lot of places, calling programs to show them you're interested, going to as many interviews as possible (and it sounds like you don't live in the US, so you will have to make arrangements to spend a few months in the US),etc.
 
A few programs that I believe are still doing pre-matches are Bronx-Lebanon, Weiss Memorial, and Woodhull.
