I went into FM wanting to do full spectrum family medicine. However now that I am looking for a job I am running into some realities and have questions .. was wondering if anyone on these boards could help ..

1. Now that I have a family and am even interested in a part time job, the idea of clinic+hospital rounding+potential nursing home rounding, etc sounds like a lot of hours and pretty stressful. (Although it would be my dream job)
Are any docs on here able to comment on whether working on a place where for example it may be rural/critical access with full spectrum work but you are still able to have a life/would be able to take care of kids? My husband works also so that is a factor.

2. In the location where I'm interested in working, most jobs, even the rural ones, seem to be clinic only, which I think is mainly due to the culture in this area of FM docs not wanting full spectrum.
There is however the opportunity to take on those roles (for example, get hospital privileges and round on my own patients or take shifts doing OB). That said.. I would be the only FM doc in the area doing that work.

Would appreciate any input!

When you say "full spectrum family medicine," are you also including OB? Which, out of the five settings (clinic, inpatient, nursing home, OB, ER), would you be most willing to give up?

If you're most interested with clinic and rounding then on your own patients then you'll likely fill your patient panel in no time. That role is becoming increasingly scarce (for pretty good reason). Expect to never have a true day off work. Ever.

To find full spectrum Fam med work, you've probably gotta go somewhere that they have a reeeeeeally hard time recruiting specialists. Replace "rural" with "the middle of freakin' nowhere." If given the choice, the vast majority of people will choose a specialist over a fam trained doc for any involved procedure pretty much every time. You can have a life in the middle of nowhere only if you enjoy middle of nowhere type activities. Finding a good school for your kiddo(s) may prove challenging.
