1. Is it worth mentioning in my secondaries, or should I just wait until it is actually submitted / published and send an update letter?

2. If I do mention it, would I cite it based on the tentative title or do I just mention that my name is on a manuscript that is going to be submitted soon. My gut tells me that mentioning it this early may not be a good thing, but its my first publication so I would really like medical schools to see a bit of the product of my work!

I think it is worth mentioning in the secondaries in the appropriate sections; although there is certain terminology to use. I can't remember the name for your case, something like, currently undergoing peer review. I, for example, have a manuscript waiting to be published and I have it cited as "in press" in my primary application. Although, if you mentioned anywhere in the primary or secondary application that you were directly involved in writing the manuscript for your research or were a primary participant they would get the vibe that you have a manuscript in the works.

If you haven't submitted to a journal for review yet, it would be "manuscript in preparation"
