I have to be honest, repeating a year from a low tier school makes matching a competitive surgical specialty nearly impossible. Even if step 1 wasn’t going p/f, it’s not really realistic to hope that you’re going to go from failing to 250. It would also be inadvisable to spend time outside class doing research which is necessary for those fields and risk failing again.

There are plenty of of other procedure oriented fields that could be attainable. But honestly right now the only focus should be on each block as it comes.

A friend of mine took a leave of absence from a mid/low tier MD school. The reason for it being initial adjustment issues that stemmed from much later acceptance and distance from campus. While his grades improved throughout the latter half, the committee that decided on her leave felt that her failing grades from a previous block put her at an academic risk. She plans on restarting next fall and made the appropriate adjustments required for her to succeed academically. What are your thoughts, especially considering the latest USMLE p/f adjustment on being competitive for a speciality like ENT?

There is redemption for your friend if he does well on Step 1 (assuming he takes it before it becomes P/F), does well on Step 2, honors as many clinical rotations as possible, creates great relationships with mentors for outstanding LOR's, and is extremely productive with research. If you struggled with preclinical coursework but kill Step 1 and the rest of med school, you will be that much more prepared to answer the inevitable questions regarding the repeated year.

Your friend should concentrate on fixing what is broken, getting back into the groove and becoming a good medical student. Specialty should not be her and your main concern at this point.
