Let go (or maybe fired?) from Tutoring 6 years ago

Hello all. I tutored (paid tutoring) at my local community college about 6 years ago , accumulating about 1400 hours over 1.5 years. Everything was great and I was one of the last tutors there who was there that long. Unfortunately, I screwed up. My boss asked me to tutor and I agreed but ended up having to back out the day before. I come into work the next day and my boss seems very "off." Somewhat ignoring me and barely making eye contact. It was time to fill out renewal forms for the next semester of volunteering and while I filled it out, I was not called back by my boss to begin working the next semester. So I called him and he explained why. I spoke to his supervisor, the head of the volunteering department , and she agreed with him (ofcourse I agree too that it was a stupid move to back out last minute). I am not sure if it this was being Let go or Fired. Should I include this experience in my application or leave it off? I can put HR as the contact info.
