For the evaluators institution, if I just put the school, do I need to fill in the evaluators addresses and city and all that? Wouldn't the name of the institution be enough? I know the address part is not required

1) Why is this urgent?
2) I think you answered it yourself with the second question.

If it will make you feel more comfortable, put the address. I'm guessing it's not going to be the dealbreaker with your app if you don't.
