Letter of Recommendation Waiver Form

Hi Guys,
I am applying this cycle and asking my profs to write letters for me. I am telling them to use the AMCAS Letter Writer Service so that they can upload the letters themselves instead of using Virtual Evals or Interfolio. Should I also be providing them with a waiver form for them to sign which states that I have not viewed these confidential letters? I don't know where to obtain one, is there a generic standard that AMCAS prefers?

When I went through this (my school had a letter service). I had to sign the waver saying that I wouldn't read my letters. I'm not sure that your writers really have anything to sign except their letters.
