If there is any chance of the ENT faculty knowing anyone who would be reading the letter, that is a plus. E. G. You are applying to the program where the ENT faculty trained. The more widely he is known, the better. There is really little chance the vascular surgeon will be known by anyone. Also, if the ENT offered to write the letter that is a good sign.

It seemed like faculty wanted to read letters from people they know: people they either are familiar through their professional accomplishments or people they trained under/with/etc. They're vouching for you. It helps for the program director to know who is doing the vouching.
