Letters of Recommendation

I have a quick question. Can someone explain to me the time-line for the letters of recommendation? For the schools that want them submitted through AMCAS, do I include the letters with my primary application? What about the schools that don't use AMCAS? Can I send my completed application in without the letters and then follow up? When is my primary considered complete?
