Listing Shadowing on AMCAS

Hey guys
When you listed the docs you shadowed...did you just group list them as one activity? When you did that, how did you fill out the contact information and the date? I have five doctors that I shadowed and I'd like to group them all together under one heading--"Shadowing"

One activity - listed them all together. Otherwise I would have run out of room for other important activities. I don't remember how I listed the contact information...
