For example, if the author is professor of surgery and chief of cardiothoracic surgery, what should I list under title?
 
Doesn't really matter, it'll be on their letterhead when they send the letter in. If you're worried about it you could put Professor & Chief, Cardiothoracic Surgery.

It doesn't really matter.

But you could always look at their listing on the hospital website and use the designation from there.
 
Pick the one that gives more weight. For instance if the writer is a clinical assistant professor and also a PD, I would list the PD title. Usually Chair or PD letters are given more weight.
