Sorry if this is an obvious/redundant question... but for LORs that are sent via snail mail, is there a form that the writer needs to attach? What's this "letter waive" check box that everyone seems to be talking about?

I think it's just for letter services like Virtural Evals and Interfolio.

It depends on what school. Some include/ require forms. If there were no forms, I gave them the letter writers a cover sheet with my name and AMCAS number, etc.

Thanks for the responses. For this cover letter, would it be in business leter format... should I address it to the school and sign it with my name? Or will that look sketchy since I'm waiving the right to see the letter and it's going to be attached to the letter itself?

Here's a example of a cover letter, try to use it as a template:

Recommendation Cover Sheet
I have waived my right of access to the letter of recommendation from the following person(s) regarding my application to the [Insert school name here].
___________________________________________________________________
Signature of Applicant must appear here if access to letter of recommendation has been waived. Date
___________________________________________________________________
Applicant (AMCAS Name) (please print) AMCAS Identification Number

Try using something like this.
 
OR this:

WAIVER

My signature below indicates that I hereby expressly and voluntarily waive any and all access or viewing rights I may have under Family Educational Rights and Privacy Act of 1974 (FERPA) and subsequent amendments, or any other law(s), to the letter of reference submitted by the author. I waive access to the letter submitted by the author listed above.
