Making IR the focus of your PS but applying DR?

I'm hoping to keep my options open by applying to DR programs with ESIR, so that if I commit to IR after some rotations, I have the option. But I would like to make my PS about IR because I had a great experience in the field recently.

Would DR program directors be confused by an IR-centric PS, given how different these specialties are in terms of personality type, work flow, patient responsibilities, etc? Thanks!

I would recommend not doing that. You should mention you like both diagnostic and interventional. But don't make it IR centric.
