matching form for transcript submission? Where?

Some one on SDN mentioned that there is a matching for for transcript submission, but I can not seem to find it even after reading the FAQ. Does it actually exist? If so, where? Thanks!
 
You'll have to "add schools" then once they are added, a small blue icon on the right hand side will allow you to print transcript matching form.
 
I had a hard time finding it, but it's there. It's a tiny blue arrow thingy. You'll see it...
 
On that section colleges attended, there is just a small window right at the begining and it says enter college or university when you click on the icon and find your college. Ok my question is since I attended multiple colleges, how am I going to add all those in that small window. I am assuming that its asking only the college I graduated from or will graduate. Can somebody explain?

Enter one college and save it. Then the page will pop up showing you that college in a standard format (that has the link to print the transcript matching form). If you scroll down from there you'll see that you can then add another college. Just keep repeating the process until you have all the schools that you have attended or will attend entered in. You have to enter any future schools you will attend in order to be able to list your planned course work.
