If a student is accepted to medical school as an honors student (enrolled in honors program), but chooses not to graduate honors, is that grounds for the medical school to revoke the offer of admissions?

For clarification, the student's GPA would remain about the same, but she wouldn't graduate honors because she did not complete a thesis. Also, "Honors Program" would not be mentioned as an EC on AMCAS, but the student's transcript would indicate having previously taking honors courses.

You are not obliged to graduate "With Honors" just because you took honors-designated classes. Even if you did mention being in the Honors Program on an EC listing, the medical schools won't care if your transcript doesn't say 'Summa Cum Laude', etc.

From my understanding, not completing an honors thesis will not affect your matriculation, as long as your grades remain at their current level. If you did mention that you are graduating through the university honors program, however, it may be a different story. In many cases, an honors thesis also satisfies writing course-requirements for many schools, which is why it is appealing.
 
The University of Utah essentially told us what Dendrite just said. If you say you're going to do something and don't, they can pull your acceptance. I think it's probably school dependent, but that's certainly true at some schools. Now, if you just have a few honors classes on the transcript, they're not going to reject you if you never said you were going to graduate honors.
 
Usually these things apply to finishing a Masters or PhD program, not graduating college with honors.
