I know this school accepts transfers into years 1, 2, or 3 of the Pharm D program, but how hard is it to get into? I'm considered a transfer student, but I haven't completed all of the pre-reqs yet. Any idea of what I'll need to be accepted?

You can talk to the school. They will go over your transcripts and tell you what year you can transfer in or what classes you need to take to enter as a year 3 student. I don't know how many slots they have for transfers, I'd imagine that it depends on how many people are dropping out.
Beware though, I have a friend who did this. She was told she would enter as a year 3, and then when the time came, she was put in year 2 because they said the organic chem she took wasn't a "medical-based" orgo, whatever that means. They have a bit of a reputation of being tuition-hungry.

What WednesdayBridge said is pretty true. I'm only first year, but many people I've talked to say that MCPHS only cares about money. As a first year, I know it was pretty easy to get into as a freshman, not sure as a transfer, but yeah they are very picky with what classes they will accept, which is wack... Be aware though, if you're looking for a college experience this place isn't for you. Also, the school is so small that it's basically like high school, if something happens you'll hear the latest gossip quickly just like in high school. There's not many thing to do at the school. Anyways, I personally do not recommend it unless you are sure you want to do pharmacy here. Anyways, just be wary about MCPHS... A bit off topic, but still relevant to the idea that MCPHS isn't that great, is that the school mistakenly thought my friend was this other girl with the same name and they forgot to give her a Chemistry II class and it was too late to get into the class, so now she has to take it during the summer and she's not getting any money back...
