MD/ PhD Applicant- Chances?

I will be applying to MD/PhD programs this summer and would really appreciate some feedback about where I stand. I'm currently trying to decide if I should add an additional year of postbac research to further strengthen my application (with pubs and further presentations). Thanks in advance for your time!

Academics:
cGPA: 3.9+
sGPA: 3.9+
MCAT: Projected equivalent of 36 based on practice tests
Full-ride + grab-bag of scholarships; Phi Beta Kappa

Languages:
Fluent in 2, intermediate in 1

Non-clinical volunteering:
~250 hours
Disparate activities

Clinical volunteering:
~300 hours
Direct patient contact
~50 hours
Recreational program at cancer ward


Teaching:
~150 hours
Physiology TA

Research:
-3+ years; poster presentations (international meetings); publication (1 first-author, 1 second-author); associated awards

All other extracurricular activities:
-Started undergrad journal club
-Editor-in-Chief of international undergrad research journal
-ER scribe

Your fate at MD/PhD programs to a large extent will be tied to how the MCAT goes.

You have many important elements to a successful MD/PhD application. Unfortunately, so do many people who apply which is a big part of the reason why the average MD/PhD matriculant(note average matriculant in general, not matriculant at a top school) has a 3.8/35. If in fact, you can hit 518+ on the real deal you will be in a very solid position. But of course hitting top 3 percentile on something like the MCAT, is a big if and easier said than done. That should be your primary focus at this point.
 
Your school list will be dependent upon your MCAT. If you hit a 515 MCAT, you can likely get in somewhere at least, 518, you can play with the big boys, and 520+ and you're highly competitive at any MSTP program.
 
Thank you all so much for the replies! I will focus the majority of my efforts in the upcoming months on killing the MCAT.
