I am applying for a medical license for the state of Indiana. I don't have any other license except for the residency "training permit" for the state of new jersey while I was a resident there. Indiana medical board asks for a "verification of licensure" form to be filled out by each state that u have had a medical license, and when I asked them if a training permit is included in that requirement then said Yes.

Has anyone else had to have verification of a training medical resident license/permit, when applying for a full medical license? I feel like this is very strange. And they ask for the "license number" but my residency coordinator says there is no number on my training permit.

Please let me know of your experiences.

Yes, this is normal.

Any medical license you've ever had (training or full, in any state) will need to be reported when you apply for a new license.

The more states in which you've had licenses, the longer it takes to get verified.
 
This is sort of a follow up question, I am a third year resident in Illinois. Now that I have completed two year of residency I am eligible for a permanent license in Illinois. I currently have a temporary license that is valid till I finish residency in two year. How would I go about getting this temporary license change to a permanent? Would I have to file a new application that would invalidate the current one I have with my hospital?

Check with the rules in IL (maybe ask a senior or somebody in your GME office) but in general, a full/unlimited license will replace, not invalidate, the limited license you have. It generally takes 3-9 months to get one so of you want it, start now.

But unless you're planning to moonlight, or your program requires it, save your money and wait until you're close to finishing residency. A full/unlimited license has no other currency for a resident.

Check with your PC and the GME office. Some states require you to have a training license throughout residency even if you have the full and unrestricted license (PA is one). Also check with your program as to whether or not you are required to have the unrestricted license: ours required it (as well as a DEA) for any senior resident. Otherwise unless moonlighting as gutonc notes, it won't be of much use.
 
You don't need a training license throughout all of residency in Illinois. I believe IL renews in 2 or 3 year cycles. To save money, you may want to wait until it's almost time to renew and then apply for the full license rather than pay license app costs twice in a short timeframe since they don't prorate (all licenses renew at the same time)....this assumes you don't NEED the permanent license now for moonlighting or other reasons.

My program also required us to get our permanent license by our PGY-3 year (but no DEA).
