Medical records sections

In academic papers there's sections like "Introduction," "Results," "Discussion," "Methods," etc. I'm looking for something similar for a sentence in my personal statement, maybe something like "Medical History," "Symptoms," "Allergies," "Diagnosis," or something like that, for medical charts. Are there standard sections and what are they? I'm not trying to look like a smartypants or anything, it's for a funny joke.
