Mistake analysis

As you guys know, mistake analysis can be quite a beneficial tool for improving on your mcat score. I want to make an excel spreadsheet to figure out what categories my mistakes fall into (unfamiliar with content, over assumption, etc). I also want to do this with the questions I got right too.

What are some common categories most mcat mistakes fall into? Successes? What categories do questions fall into?
