Id have to respectfully disagree to the comment about CVS system only revolves around profit and not safety. Coming from a small pharmacy and going to CVS I really feel that the system has a lot of safety measures put in place to avoid these errors from filing with wrong prescriber to dispensing wrong drug. The system does have its glitches however it does prevent a lot of mistakes from happening. At the end of the day the major responsibility is placed on us as the pharmacists and with or without a system it is our job to make sure everything is correct and we shouldn't solely rely on as system to do our duty. There is no system more accurate than your own eyes and hands.

Yeah, and I'm gonna have to respectfully disagree with you as well. It's not the system that makes CVS unsafe, it's the environment they create through inadequate staffing, incompetent technicians and lack of support on a system level that make CVS a danger to society.

It doesn't matter how great your system or workflow is, if it's designed for 5 people and you supply 3 people you are adding strain, stress, distractions, and interruptions which significantly increases the chance of error. I would also argue that many systems have too many alerts, which results in "alert fatigue". If it becomes second nature to override insignificant alerts than the alerts that actually matter become less attention grabbing.

My main point, though, is that inadequate staffing is the problem.

Yeah the staffing is pretty bad. Today was an awful day. They really need to allow more people on staff for the demand we get but I know that's never gonna happen in this lifetime
 
CVS is going to get worse and worse for pharmacist. They are adding more and more responsibility with less help. Now with verify on holds and mandatory counsels going to QV, it just might be too much for some people. CVS is going to have to pay top dollar to keep people
