There is so much self selection before and during medical school.

Women think if they want to do surgery they have to choose between a family and work. Several of my classmates are super vocal about "never" wanting to date a female physician, let alone a surgeon. They want wives to cook and clean.
Are there exceptions? Sure, but the prevailing attitude is the above.

Compound this with being told constantly, "you should really think about the lifestyle for you and your family" to " you will never be an orthopedic surgeon" (happened during my third year, from a female physician) it is not hard to see why women aren't clamoring to go into surgery.
