Most important criteria for MATCH?

Just wondering what the most important criteria for matching is. When you look at a lot of match lists, I often notice that most people end up in program close to where they went to school. What is this trend due to? If you want to match at a residency which is not affiliated with where you went to med school, how much of an uphill battle will it be to land a residency elsewhere? How much do inter-instutional connections affect the match? Thanks in advance.
