Applying into Neuro, but was gonna do IM/FM/EM. My most meaningful experiences in truth are leadership in IM interest group (gained a lot of skills, lead meaningful events), basic science research I did in undergrad through my first year of med school, and volunteer work in the community. None of these are related to Neuro directly, but are most meaningful to me. Should I be honest and do these 3, or should I change one to a Neuro experience?

It's important to balance being genuine with box checking. The best applications tell your story. It's perfectly okay to have an interest in another field. I recommend focusing on what's important to you, but consider replacing one of your current interests with something related to Neurology. This will give you something more recent and relevant to the field you're applying for. Not including anything related to Neurology might be seen as a minor drawback by some reviewers and you have to cater to all audiences.
