Need guidance on NBDE process

Hi All,
I want to pursue dental career in US and I am currently practicing in India with my B.D.S degree. I understand that I need to take NBDE part 1 and part 2 and then submit application to schools to get the DDS degree from US university. I have following question about NBDE:

1. Are there any preparation courses offered in US or India (mumbai).
2. To take NBDE which visa I need to apply for?
3. How does the Indian degree evaluation works?
4. How soon I need to submit application for NBDE?

Please correct me I am wrong in my understanding any guidance will help.

Can you please give me website for courses in India? Link you have given is not working...also what is general trend of getting visitor visa to appear for NBDE in US?
