need info regarding fellowships

hi,

i have been a avid reader of this forum. i am an IMG and finished my MD in internal medicine outside US. i have cleared MRCP part I exam. i have cleared USMLE step I and II and will be applying for step II CS in some time.

i am interested in direcr fellowship in US since i am planning to come back to home country after gaining experience and exposure in US.

i am interesed in neprology as my first option and then others being endocrine and hematology/oncolgy.

i wanted to know what r my chances in getting my fellowships. i have quiet a few publications, presentations in conference, my scores were ok that great like 85 and 88 in step I and II respectively.

i would also like to know if i can take american board exam for internal medicine directly without doing the residency......

pls also help me in finding good website which will guide me in this endeavour....

Hi Hari,

In order to get a nephrology fellowship (or Endo or Heme/Onc) in the US, you have to do three years of Internal Medicine (regardless of what you have done anywhere else in the world, even if you have practiced Nephrology for 50 years in the UK or India) and then apply for a fellowship. Some hospitals may let you start in the second year of Internal Medicine but you cannot skip that part altogether.

Nephrology in general is not a very competitive fellowship, so as long as you can get an Internal Medicine spot, you should be ok.

I disagree with the above statement. Some US fellowship programs take foriegn trained physicians who are eligible for state licensure with J1 visas.( passed step1 and step2) You dont have to complete your internal medicine residency in the US. You have a chance in least competitive fellowships not filled by US candidates. I think you should apply after completing your MRCP part 2. You cannot take american board exam without US residency training.

I know many folks who have done surgical fellowships after FRCS. same rules apply for MRCPs.

newyorker is correct. You do not have to have completed your residency training in the US to be eligible for a US fellowship. As a matter of fact, many fellows, especially in the super-specialties (which may not be as competitive due to their narrow focus) are internationally trained. Half of the 16 hand fellows taken each year at the Kleinert in Louisville, KY have completed residencies outside of the US.

To be Board Certified by the American Board of Internal Medicine, you must (from the ABIM website):

Certification requirements include:

Completion of the required predoctoral medical education
Completion of required training in an accredited residency program
Assessment and documentation of individual performance from the residency training director or from the chief of service in the hospital where the specialist practices
An unrestricted license to practice medicine
Passing a certification examination
Candidates who have passed the exams and satisfied other requirements are then given the status of "Diplomate" and are certified as specialists.

This would presume training in a US program but may not be necessarily so. If the Board was willing to approve your foreign IM training, you might be eligible.

right, the rules are ambiguous, but realistically, i've never heard of getting IM fellowships w/o IM training here. i have no info on surgery so can't comment on that field.
