Negative mark on record, what is my chance?

So I have a case in my record where I was responsible for using a fake parking permit. I wasn't intending to use a fake permit but that's a long story. Summary: I explained it to the school that I didn't know I wasn't supposed to use the permit at the time and not like I wanted to fake a permit or anything.
But anyway...
It stays on my record. Should I add this to my personal statement and talk about it? And how is my chance affected by this?
I currently have >3.8 GPA. Some volunteer and leadership experience.

This is not necessary to report. This will not negatively influence you at all. If by some whack chance you get asked about this explain it in person then and I'm sure they will be understanding.
 
I did receive a written warning about this. On pharmcas it says to list all types of misdemeanors that I've had. Should I list this?

If you weren't convicted through a criminal court then you didn't commit a misdemeanor. .

I got a written warning for partying in undergrad dorms back in the day .. it was on my "record" that is.. in some file somewhere at some school.. it never came up again. I didn't declare it. If it had been a criminal charge such as underage drinking then that would be different. If police weren't involved, you didn't commit a crime.
