Apologies for my wordiness (and possibly dumb questions). I had a few questions about the New Jersey medical license application I am hoping you all can shed light on. I plan to call their office once it is open as well but am hoping to get a quicker answer.
1) Under section 1/Personal Information, consent to use SSN for various listed purposes: Is there any consequence to saying "no?" They effectively say (to paraphrase) "this is voluntary and you can't be punished for saying no," so why would any one answer yes? Just wondering to be informed ...
2) Under section 1/Address Information: what did you use for "address of record," which is both published and printed on the license (as opposed to mailing address which is kept private)? I'm listing the only public address I have to provide -> my current (non-New Jersey) employer but this will be obsolete by the time I am using my NJ license.
3) Under section 2/Medical Schools: When entering the medical school information, the first field says "year" and the dropdown menu lists the numbers 1 through 6. This is a mandatory field. What is this asking of me? I went to one medical school and took a brief leave of absence (so start to finish was 5 years), so do I answer 5?
4) Supplementary documents: The summary page only asks me to upload a photo, birth certificate/passport, verification that my current/previous licenses have all been in good standing, and (if applicable) docs related to name changes/criminal hx. Somewhere else, there was mention of uploading a CV. Did you all provide an official CV in addition to the application? I of course already know about BME-MEV/VPT/PEA, transcripts, and criminal background check.
5) The public website (not online application) has a PDF for malpractice insurance verification form but at no point does it ask for this form to be completed (it asks for all of the other 4). Is this only if one has been sued? It's actually not clear to me from the form itself either ... and I have no idea where to take this to get completed. My employer is a national organization based in a different time zone than me so I can't go to their offices ...
6) Can the criminal background check/fingerprint process be completed remotely (I'm in the Midwest)?
Sorry for the nitpicky and wordy questions - I'd rather sound dumb than miss something that delays my application. Thanks in advance for any tips!
EDIT: Just after signing the above, I found the PDF instructions for the paper application. Seems a few details were left out of the online application instructions. Seems I should include my CV and malpractice form is for any agency covering me in past 3 years regardless of litigation history (makes intuitive sense too). And they send you fingerprinting options if you do not live near NJ state. I grayed and italicized those questions since I can't strikeout but you're free to comment if you have input anyway! The difference in detail between states throws me off (NY for example asks for hardly anything!)
