I think chronic passive congestion of the lung is a result of fibrosis/thickening ... I may be wrong though

I have a question regarding the ethics ...

A mentally ******ed patient come to the clinic and the mother of the patient asks the dentist to replace the patient's amalgam restorations with tooth coloured filling but the patient disagrees and is non cooperative for the treatment.what will be your decision?

1.disregard the patient and go ahead with treatment.
2.consider the patient as autonomy and do not replace the restorations.

i feel that if the restorations are ok,then no need to replace but if these are faulty ones,then they need to be replaced..

So how does informed consent come into play in this situation given the fact that the patient does not have the mental capacity to make an educated decision. Does the final decision rest upon the legal guardian?
