After a student fulfills their NHSC obligation as a primary care IM doc, can they then go back and subspecialize if they realized they didn't like primary care? And if so, how difficult would it be to get into a desired fellowship?

Yes, you can go back and subspecialize AFTER you fulfill your service payback requirement.

As to how difficult it would be to get a fellowship after 2-4 years as an attending, that will depend on what fellowship you're shooting for. Rheumatology or Allergy/Immunology? Probably not that tough. GI or cards? Probably very difficult.
