NICU - Stat order form... What are your hospitals using?

We are stream-lining the stat order process on our neonatal ICU service.

To expedite the ordering of stat meds for a patient not-yet in the system, we are creating a form that can be faxed to the pharmacy with standard concentrations.

Right now, the medications that will be included are:

Dopamine
Dobutamine
Epi
Nor
UAC/UVC fluid (1/2NS + 0.5unit hep/ml)
Vanc
Gent
Cefotaxime
Alprostadil
Indomethacin

I'd appreciate any thoughts / opinions on what should be added or removed.
 
I'm in a smaller hospital that usually ships out sick newborns pretty quickly. So we usually only get to send the IV fluids and antibiotics before they leave. How do you enter orders for a patient not yet in the system?

Your standard IV fluids will probably depend on your MDs? Our place tends to always go with D10 + 100mg calcium gluc/100mL + 1:1 heparin. And our common stat antibiotics are cefotaxime and ampicillin

Are they having you do this after what happened at bamc yesterday?
