For South Dakota I printed the application, from their website, and sent it in by mail. I'd assume that's how others are done as well.
I believe this is how it's done for any school not using an online application, in any field. Some places will accept faxes, since carrier pigeons went out of style.
If you can't find the app or can't figure out how to get it back to them, call the school's admissions area (guessing all their apps are paper if they don't use PharmCAS) or the college of pharmacy directly.
