Other suggestions??

Did you spend your childhood primarily in rural communities? Aside from state residency, have you checked out whether the schools on your list have rural health tracks?
I did not grow up in a rural area, but I live in one now. The clinic I currently work in serves patients primarily from small town/rural VA and WV. I know that IU, WVU, and MCW have rural tracks. I am not sure I have strong enough rural ties to convince schools with rural tracks that I am committed to rural medicine, so I don't want to limit myself to schools with rural tracks. Moreover, I want the option of specializing, and it seems like many rotations in rural tracks are limited to FM, peds, OB, etc.

You could add any of these schools to your application:
Dartmouth
Tufts
Boston University
Brown
Hofstra
Einstein
Mount Sinai
NYU
Pittsburgh
Cincinnati
Columbia
Cornell
Vanderbilt
St. Louis
Tulane

Dartmouth
Tufts
Boston University
Brown

NYU
Pittsburgh
Cincinnati

Cornell
Vanderbilt
St. Louis
Tulane
Add Yale, Harvard, U MI, U VM to list
Thank you both very much for the feedback. I have incorporated your suggestions (based on geographic preferences--I would rather not be in NYC) into a revised list below, with a few of my own additions in bold.

Does this look reasonable or too top-heavy? Should I add more lower tier schools (Drexel, Albany, Rosalind Franklin, etc.)?

Case
CCLCM
Cinci
Dartmouth
Duke
EVMS
GW
Gtown
Harvard
IU
Mayo
MCW
Pitt
Tulane
U MI
U VA
U VM
VA Tech
Vanderbilt
VCU
Wake Forest
Wash U

WVU
Yale

Do not apply to Drexel, Albany, or Rosalind Franklin. They are low-yield and may not consider you with your high stats.

You can also apply to Ohio State, Jefferson and Rochester (completely different part of the state than NYC).
