Could someone explain this question to me I don't understand why D can't be a possible answer choice as well.

This question is almost CARS-like: it is testing (among other things) whether you can make a prediction based on only the information that you're given, rather than picking a possibility (like in choice D) that is not impossible based on the question, but is also not directly supported. In other words, the idea is that the question says "blah blah blah ↑NADH" and the correct answer choice says "↑NADH means ↓gluconeogenesis." That's it. We don't know what the process is that generates ↑NADH, so we can't make any prediction about plasma glucose levels. Hope this helps!
